The use of drug-eluting stents (DES) in complex cases has been a subject of controversy since the 2006 FDA safety hearings concluded that there is a slight increased risk for late stent thrombosis with DES vs bare metal stents (BMS) when used off-label. The SYNTAX trial suggests that DES can hold it’s own against coronary artery bypass graft surgery in patients with multivessel disease -- at least in terms of hard outcomes. The data for DES use in acute myocardial infarction (AMI) are mixed with randomized trials showing no mortality difference for DES vs BMS while some registries show an increase in mortality and others show a survival advantage. Recorded in Washington, DC at TCT 2008, Drs Kandzari and Bhatt each lead a case-based discussion on the use of DES in multivessel disease and AMI drawing upon recent data including HORIZONS-AMI.
